Hello! I was searching on internet at Los Angeles Superior Court and I found phentermine is already as part of the Civil forms application for claims..

I went for a general check out to my doctor, and he told me not to take ever again phentermine is really bad for the blood pression and big risk for heart attack.. That document looks to be part of the FenPhen (or whatever it was called) situation. Phentermine was not the bad part of FenPhen.
Other than that, I don't know what that document is... it seems to verify that whoever was in charge of mailing something knew the date it was supposed to be mailed... |

Thanks so much guys for the information..I didn't heard about FenPhen.. My regular doctor (not the one who gave me phentermine) told me not to continue with phentermine because is danger for our heart and our blood preassure. I didn't check my heart, but the time I was taking phentermine I felt as my heart was beating so fast, I could heard it at night when I was sleeping.. ;(
