dr olivera in chicago at illinois masonic www.olivera.com. they are super nice and it's 85.00 a month including all drugs and b12 shot. they also give 10.00 for referals to existing and new patient.

Actually, Dr. Olivera's office has been giving out coupons for $10 off to all patients (to be used at the next visit) so visits are only $75 if you remember to bring the coupon. And, if you bring or send in a new patient, you can save an additional $10. I only paid $65 last time and that includes the Phentermine. :)
